It's quick and easy to use the MyWellSpan portal to access and view your health information — any time, anywhere. To request a copy of your medical record, follow these steps:
- Under the menu, go to Sharing > Request Medical Records.
- Provide the requested information.
Alternate method: paper form
If you’re not able to use MyWellSpan, you can print and complete the Authorization to Use or Disclose Health Information form, available in English, Español and Haitian Creole (PDF).
Follow the instructions at the bottom to mail or fax your completed form.
Request a change to your medical record
Best method: your MyWellSpan account
With a few clicks in the MyWellSpan portal, you can instantly submit a request to change your medical record:
- Under the menu, go to the Document Center.
- Click on Requested Records & Record Amendments.
- Select Complete a Request for Amendment Form.
- Provide as much information as possible. This includes the type of documentation (progress note, discharge summary, problem list, etc.) and the date of the documentation.
Alternate method: online or paper form
If you're not able to use MyWellSpan, you can complete the Patient Requested Amendment Form (English - PDF) or Español (PDF) to print and mail your form to:
